Carcross/Tagish Mission Statement

Your Role
Reporting to the Director of Health and Wellness, the Elder Care Worker will be responsible for integrating the four elements of Values and Virtues of Life (mental, emotional, spiritual, and physical) into the on the land family and children supports provided by Family Council.

Duties and Responsibilities
- Maintain regular (bi weekly at the least) contact with all clients and soon to be clients; contact will fluctuate as client needs or circumstances changes;
- Document all interactions with clients on their individual client files, maintaining both physical and electronic files;
- Ensure that clients are notified promptly of community events and opportunities, C/TFN communications intended for clients/citizens/community members, Federal or Territorial changes to services, especially as it relates to NIHB or senior services, Health and Wellness specific initiatives, or other such communications where the client can benefit spiritually, emotionally, physically or mentally;
- Assist clients as requested by the client, by way of assisting the client to complete forms, applications, letters, use the internet and other devices to make their life easier, and other relevant assistance;
- Advocate for all clients who may need the support, this may include researching, attending appointments, assist phone calls, make connections, etc.;
- Develop, implement and facilitate workshops and programs for clients/citizens/community members that are aligned with the values and virtues of C/TFN;
- Arrange for monthly transportation for elders to attend Whitehorse for grocery shopping, appointments that may not be medical in nature, and other errands;
- Work with family members of clients to coordinate areas that the client is having difficulty with, ensuring confidentiality and privacy are open as possible, and closed as necessary;
- Administrative duties, not limited to statistical reporting each month and file maintenance
- Ensure items in the Elder’s policy, or other applicable policies, are followed to ensure good service provision (such as coordinating heating fuel to elders);
- Coordinate Elder’s Council, an advisory committee; meetings, minutes, honourarium, and transportation;
- Managing and maintaining comprehensive client files and records all interactions in case management system (such as ShareVision), that are compliant with all current legislation and policy;
- Positively and professionally represents the Health and Wellness Department, and C/TFN with colleagues, citizens and community. Establishes and maintains effective working relationships;
- Demonstrates a commitment to personal growth and development to remain current and stay abreast of developments and opportunities and to exchange information;
- Assists with reporting obligations related to program funding;
- Complete all other duties as directed by the Health and Wellness Manager and the Director.
Essential Qualifications

- Knowledge of the communities of Carcross, Tagish, Squanga and Whitehorse, and C/TFN citizens
- Good understanding of determinants of health and well-being for elders and vulnerable people;
- May need to shovel snow, steps, and pathways for safety and may need to stack firewood for elders.
Desired Knowledge, Skills and Suitability
- Significant knowledge of C/TFN Family Act, Book 1 & 2;
- Must be able to incorporate the Values and Virtues, and traditional knowledge of Carcross/Tagish into all events, programs, curriculum and interactions;
- Ability to work effectively as an integral part of a team and foster a team approach to service delivery;
- Oral and written communication including presentations, briefings and reporting;
- Strong organizational and time management skills;
- Ability to work with minimum supervision;
- Ability to develop, coordinate or facilitate workshops.
Conditions of Employment

- Valid Yukon Class 5 drivers license
