  1. Job Summary

Reporting to the Client Service Case Manager (once filled; reporting to the Director of Health and Social Services in the interim), the Team Lead is responsible for the ongoing planning, implementation, and administration of the Reaching Home program, while also taking on team leadership responsibilities including staff supervision of the Family Support Workers, program development and delivery, and collaboration with the Office Manager on financial and reporting matters. This position is also responsible for monitoring community needs and, where appropriate, modifying programs in the areas of support and prevention. The Team Lead is responsible for staying current with regional needs assessments, territorial and federal strategies, and other relevant strategies. As well, this position requires ongoing formal reporting, research, and evaluation.

Main Duties & Responsibilities:

  1. Reaching Home Program Delivery:
  2. Plan, implement, and administer the Reaching Home program in accordance with TTC's case management practices;
  3. Provide health-related case management for clients accessing the Reaching Home program;
  4. Provide front-line lay counselling and outreach support to TTC citizens through the program;
  5. Work with clients to prepare for and support transitions related to housing stability and after-care;
  6. Design, develop, and deliver program-related education, workshops, and events to TTC citizens;
  7. Staff Supervision:
  8. Provide dedicated, regular supervision to the outreach staff, including planning and follow-up review of casework;
  9. Support the professional development, training, and day-to-day guidance of outreach team members;
  10. Maintain an open dialogue with TTC Health and Social team members, including cross-training and regular intake meetings, to ensure timely and coordinated service delivery in the absence of a client's primary outreach worker;
  11. Program Development and Delivery:
  12. Lead the ongoing development, evaluation, and improvement of Reaching Home and related outreach programming;
  13. Stay up-to-date on outreach program best practices;
  14. Design practical and realistic programs that reflect Haa Kusteeyi for Teslin Tlingit community members;
  15. Financial and Reporting Collaboration:
  16. Work closely with the Office Manager on program budgeting, financial tracking, and expenditure reporting for Reaching Home and Family Support;
  17. Complete written reports on all program and event activities in a timely manner, as required by the Client Service Case Manager;
  18. Degree or two-year Diploma in Social Work, social services, or community/human services fields, or an equivalent combination of education, training, and lived experience in the field of addictions in a First Nation community;
  19. Minimum of 2 years' experience providing client services, case management, and individual/family support, with demonstrated experience working with First Nations people;
  20. Experience supervising staff and/or leading program development and delivery is required for this position;
  21. Mandatory requirement to maintain strict confidentiality
  22. Yukon Class 5 Drivers License

Education & Experience:

  • Degree or two-year Diploma in Social Work, social services, or community/human services fields, or an equivalent combination of education, training, and lived experience in the field of addictions in a First Nation community;
  • Minimum of 2 years' experience providing client services, case management, and individual/family support, with demonstrated experience working with First Nations people;
  • Experience supervising staff and/or leading program development and delivery is required for this position;
