Permanent, Full-time, in Burwash Landing, Yukon
Salary: $78,278.71 to $106,434.63 per annum based on 70 hours bi-weekly (depending on qualifications and experience)

Overview
This position is responsible for coordinating and delivering home support services that promote the health, safety, independence, and well-being of KFN Elders and community members requiring assistance at home. The Home Support Coordinator assesses client needs, coordinates support services, care, activities, and provides guidance to Home Support Workers. The position fosters positive relationships with Elders, families, health care providers, and community partners to ensure services are delivered in a respectful, culturally appropriate, and client-centered manner.
Education and Experience
A certificate or diploma in Home Support, Personal Support Work, Community Care, Health Care Assistance, or a related field is required. Experience providing personal care, home management and respite care in a home care or a community-based setting is required. Basic computer skills are required. Experience supporting Elders, individuals, and families is preferred. Preference given to candidates with experience working with a First Nation government or within a First Nation community. An equivalent combination of education and/or experience may be considered.
Conditions of Employment
- A valid Yukon Class 4 driver’s license or ability and willing to obtain within three (3) months of employment and an up-to-date clean driver’s abstract
- Vulnerable sector and Criminal Record clearance
- Clear TB screening
- A valid First Aid Certificate, or willing to obtain within the first three (3) months of employment
- WHMIS or ability to obtain within three (3) months of employment
- Food Safe Level 1 (completed within the first two (2) years of employment)
- Willing to travel within the community, to Whitehorse, and to other communities and to work weekends and overtime
