Health Navigation Assistant

Position Summary
Queer Yukon is seeking a Health Navigator Assistant to assist the Health Navigator in advocating for 2SLGBTQIA+ individuals and families in accessing and navigating health and social service programming. This role is based in Whitehorse, where the Navigator will be available during drop-in hours to meet community members directly and help connect them with the care they need. The work is practical, people-centered, and often rooted in everyday problem-solving, whether that’s helping someone book an appointment, understand their options, or feel supported in pursuing gender-affirming care. It’s a role that combines advocacy, relationship-building, and hands-on support in a community setting.
This is a public facing job, the incumbent is required to work from the office.
Location: In-Person; QYS office, Whitehorse
Hours: Part-Time between 24-32 hours per week (primarily weekday hours, occasional weekends, evenings or drop in hours)
Compensation: $39.51/hr to $42.77/hr (Commensurate with experience)
Benefits: As per QYS Union Collective Agreement
TERM: Term position until March 31, 2028 (Possible extension dependent on funding)
REQUIREMENTS:

- Vulnerable sector security clearance
Key Responsibilities
- Build and maintain respectful relationships with 2SLGBTQIA+ individuals, community members, leaders, and organizations.
- Working with both the Health Navigator and Outreach & Communications Coordinator, plan outreach activities, events, and workshops that promote awareness, visibility, and inclusivity.
- Support Public facing pride events during Pride.
- Deliver educational workshops on topics such as HIV prevention (including PrEP), STI prevention, and gender-affirming care.
- Assist clients in developing care plans and accessing healthcare, social services, legal support, and mental health resources.
- Coordinate appointments, referrals, and follow-ups, with Clinical Supports and updating referral lists regularly.
- Assist clients within the healthcare system to ensure they receive respectful and affirming care.
- Maintain accurate and confidential client records.
- Assist with promoting harm reduction strategies related to sexual health and substance use and education.
- Participate in ensuring safe access and maintenance to Queer Yukon Society office as required.
- Other duties relevant to this position as assigned by the Executive Director or Board.
Qualifications:
Education & Experience
- A minimum of 2 years of experience working with 2SLGBTQIA+ individuals is required.
- Experience working with individuals at high risk for HIV/STIs or those seeking gender-affirming care.
- Experience with documentation, data collection, and client tracking systems.
Skills & Knowledge
- Strong interpersonal, written, and verbal communication skills.
- Ability to work independently and collaboratively in a team setting.
- Organizational and time management skills, with the ability to meet deadlines.
- Understanding of issues regarding 2SLGBTQIA+.
